What I’m Reading This Week

I’m increasingly convinced that those who are genuinely serious about improving their heath, fitness, and maximizing their functional longevity need to invest some time reading good science and the musings of the thought leaders in the game.

There’s a whirlwind of activity in the nutrition arena these days; I hear you when you say there’s a lot of noise out there – there is – and it does take a bit of effort to separate (gluten warning) the wheat from the chaff. Keep working at it; the more you read, the better you’ll get at sorting out the useful nuggets from the bullsh*t.

My wife and I (and our adult offspring as well) have always been voracious readers; personally I’ve recommitted to a goal to read 80 books this year, and will be sharing what I’m reading weekly just for the hell of it. This week’s reads include…

The Paleo Diet, Loren Cordain, PhD (I realized this past week I’d never read his original works, and ran across a copy of the revised edition [2011] in a used book store.) He’s obviously one of the respected leaders in the Paleo world.

Two Prime Examples of A Traditional Medical Swing and A Miss: Practice Guidelines for T2DM and Obesity

As a former clinical physician I’ve always been fascinated by the promulgation of clinical guidelines, ostensibly crafted by leading minds in a given field convening to articulate treatment and diagnostic pathways to follow based on, what we’d all presume to be at least, the latest and greatest empirical evidence.

The overwhelming challenge is of course that human pathophysiology varies widely from person to person, given their unique genetics, history, constellation of conditions, nutrition, and even behavior. One-size-fits-all guidelines are often somewhat muddled in real-world application, and can lead to inappropriate, sub-optimal, or even harmful treatments being offered.

I personally know physicians who would be offended at the paragraph above, yet examples are stunningly commonplace of clinical guidelines run amuck today. For example, I have an 86-year old family member who’s currently being pressed to take statins for an incorrectly interpreted lipid panel, with her internist refusing to pursue the most specific testing, reporting it’s not allowed in his employer’s guideline directives.

For further example, here are two clinical guidelines released in the past few weeks that miss the mark impressively.

From Canada, T2 Diabetes Clinical Practice Guidelines for Physicians

From the Canadian Family Physician, Diabetes Canada 2018 Clinical Practice Guidelines, was published in January, detailing an intensive and comprehensive, pharmacologically-focused treatment pathway with stunningly little discussion of dietary guidelines focused on rational management of carbohydrate intake.

The article was presented on MedScape here (free account required); as always the comments were illustrative (and saddening), with one RN insisting that T2DM is best managed by increasing complex carbohydrates and limiting protein, a stunningly incorrect approach. Critically these articles referenced failed to address diet/nutrition in any detail, which has been increasingly shown to be impressively effective in management of T2DM.

Obesity Management in Primary Care

The second example is this: European Practical and Patient-Centred Guidelines for Adult Obesity in Primary Care.

The recommendations are multi-faceted and listed in some detail; they to their credit suggest pursuit of a “typical balanced Mediterranean style diet”, though suggest that at least of quarter of caloric intake come from starchy foods including cereals, legumes, and grains, with robust consumption of fruits as well, despite ample evidence suggesting these very foods can be obesogenic for many.

Of particular interest are the recommendations regarding targeted weight loss objectives (5-10% weight reduction), which while helpful would be woefully inadequate for many, and the responses on the Medscape summary of the guidelines, including one physician claiming the obesity epidemic to be unstoppable/untreatable.

The Take Home: Don’t Be Distracted By One Size Fits All Guidelines

Two Points of Interest. First, one-size-fits-all clinical guidelines, whether written by the AMA (or some other bastion of traditional medicine) or your favorite alternative health guru, need to be interpreted for you personally with great care.

Some recommendations you’ll come across, like the two examples above, have such glaring foundational flaws (missing fundamental nutrition and diet principles), one doesn’t have to venture too far out on the proverbial limb to suggest genuine harm is being done to many as they’re overmedicated, passing time waiting for complications of their lifestyle ‘diseases’ and treatments to manifest.

Some recommendations you’ll come across, and I’m thinking social media driven “advice” here, touting magical ingredients, supplements, workouts, etc, are not only anecdotal, but occasionally frankly duplicitous, evening bordering on the absurd. The keto world on social media is a hotbed of “experts” right now, prompting Mark Sisson to even refer to folks doing “caricature keto” in this post covering The Curious Phenomenon of Keto Crotch.

You physiology is unique to you; pay attention to what your body is telling you, and find supporting team members, including a good physician(s) when needed, who are willing to do the same.

And secondly, when anyone offers you recommendations based upon their guidelines, it’s absolutely fair game to apply several common sense tests: Do the recommendations make sense? Do the recommendations start with the least risky intervention or activity? What potential risks (or comorbidities) are involved?

An easy example – it’s my opinion that treating T2DM with pharmacological agents without a well-thought out, well-designed interval in a low carb (or VLC) diet trial is, given the information available today, clearly inappropriate. Another easy example is the liberal use of statins in clinical practice today, without adequate trials of dietary modification, particularly given statins’ long list of problems/side effects and questioned efficacy in reducing all-cause mortality in populations without proven heart disease, the risks very well may outweigh the benefits (more on statins to come another day).

Metabolic Syndrome: Diagnostic Criteria and More

Yesterday I shared a bit about the metabolic syndrome (scroll down to read), the constellation of risk factors that are deemed to represent a critical risk for developing other serious medical issues over time – including cardiovascular (heart) disease, cerebrovascular (i.e. strokes), diabetes, and several more.

Let’s take a more detailed look today at the diagnostic criteria for the metabolic syndrome and the most common symptoms it produces.

Diagnosing the Metabolic Syndrome

By the standard criteria in place today, you’re considered to have the metabolic syndrome if you have at least three of these five key criteria:

Waistline measurement of > 40 inches for men, > 35 inches for women.

Fasting triglyceride level above 150 mg/dl.

HDL (high density lipoprotein) level < 40 mg/dl for men, <  50 mg/dl for women.

Fasting blood glucose level > 100 mg/dl OR are taking glucose-lowering meds.

Blood pressure 130/85 or higher OR are taking meds to lower your blood pressure.

While not considered a diagnostic criteria, many people also have the skin changes of acanthosis nigricans (darkened skin under the arms or on the back of the neck) or skin tags.

Common Symptoms of the Metabolic Syndrome

Somewhat ironically, there aren’t any immediate symptoms associated with development of the metabolic syndrome; these changes develop over time, allowing our often over-developed sense of personal denial to strengthen as well.

An expanding waistline is surely a readily visible sign, particularly when it’s prominently deposited on the trunk and abdomen, though a person could be “skinny as a rail” and still have hypertension (high blood pressure), a high fasting blood sugar, high triglycerides, low HDL and be labeled as having the metabolic syndrome.

If in doubt, consult your physician, assess your risk factors with simple lab analysis, and chart your course for recovery.

Steps to take in dealing with a “diagnosis” of the metabolic syndrome will follow.

Metabolic Syndrome: What Is It?

Yesterday I had the pleasure of sitting down with a delightful couple here in the DFW metro for coffee and a chat about their coaching needs.

The wife had recently been scolded by her physician and told “she was a walking metabolic syndrome disaster”, and left to an apparently under-equipped physician’s assistant to discuss it further, which consisted by report of being handed a three-page pamphlet and being told to “lose weight or you’ll need a (weight loss) surgery…”.

They were (quite reasonably) aghast at the interaction, and intimidated by what her “metabolic syndrome” implies. What follows here is meant to serve as a quick introduction to the issue, not an exhaustive discussion in critical detail.

So Just What is the Metabolic Syndrome?

At the most basic level, the metabolic syndrome is the name given a group of risk factors that increase an individual’s risk for a number of bad actors, for example, both cardiovascular (heart) and cerebrovascular (stroke) disease as well as diabetes.

The American Heart Association estimates that as many as 34% of American adults currently meet the criteria to have the metabolic syndrome, an astounding number when you think about. On the other hand, when you ponder how overweight and inactive a large portion of the population actually is, perhaps it shouldn’t be all that surprising.

While not part of the absolute diagnostic criteria for the metabolic syndrome, most authorities (correctly) implicate insulin resistance, a frustrating consequence of eating the Standard American Diet for many, in the development of the metabolic syndrome.

Not all physicians agree that the metabolic syndrome should be viewed as a distinct condition, and you might hear or read it described by other names, including the mysterious sounding Syndrome X, the insulin resistance syndrome, and the dysmetabolic syndrome.

Metabolic Syndrome Risk Factors

There are host of risk factors for developing the metabolic syndrome, here are some of the most common:

Overweight/Obese. Living above your ideal body weight, particularly with abdominal fat stores, is a major risk factor.

Age. Similar to many other health bugaboos, risk increases with age. Life choices in your 30’s really do matter; by age 60, the risk of developing a metabolic syndrome tops 40%.

Diabetes. Risk for developing metabolic syndrome goes up with a family history of T2 diabetes or having developed gestational diabetes during pregnancy for women.

Race. At least in the United States, Mexican-Americans, African-Americans, Asians, and Native Americans are considered to have higher risk.

Sex. Women have a slightly higher risk of developing metabolic syndrome than men.

Smoking. While perhaps not directly causal in the metabolic syndrome, tobacco use clearly worsens the health consequences.

Insulin resistance. A critical and expansive topic to be covered on its own; it must be included on any list of metabolic syndrome risk factors.

Other medical conditions: Non-alcoholic fatty live, polycystic ovary syndrome, cardiovascular disease, certain types of biliary disease (gallstones), lipodystrophy, and more.

Today’s Friday Feast is this amazing Hanger Stea Today’s Friday Feast is this amazing Hanger Steak Bulgogi Bowl (recipe posted on the blog this morning, link in bio). Hanger steaks, like their kissing cousins skirt steaks, derive from the diaphragm though are much more tender and flavorful than skirt - which is still our favorite #fajita cut on the planet. I put this bowl together last weekend before heading down to Austin with our Asian Fried Cauliflower Rice; hot damn, was it tasty. And it’s a simple prep and cook to boot.
